8 October 2025 | OV | Harrogate | The IP was assembling the rotary sieve which involves applying a clamp which secures the two decks to the sieve together. While the Ip was closing the clamp with his left hand, he was using his right hand to hold the clamp in the correct location on the opposite side of the sieve. As he was closing the clamp, the end of the IP’s middle finger got nipped in between the clamp and the sieve causing a small section of skin to be removed – approximately 4mm x 2mm. | |
11 July 2025 | OV | Harrogate | The IP turned the transfer pump on the Scotsman line to pump product through the plate pack, opened the valve to ensure toffee and not water was being pumped and it spat hot toffee onto the floor which splashed the IP’s right forearm. | |
17 June 2025 | OV | Harrogate | The IP was carrying out the task of filling the sugar tote, that was empty, with a 1 tonne tote bag of sugar. It had run out just before he was going for his break. The IP lifted the empty sugar tote and frame from its stand and lowered it to the floor with the FLT. He then brought a 1 tonne bag of sugar from the warehouse into the factory and lifted it above the sugar tote with the FLT. The next stage was to undo the two strings on the outlet chute on the bottom of the bag to release the sugar into the tote. | |
30 July 2024 | OV | Harrogate | The IP was un-winding a conveyor belt from a roller on a new piece of machinery with a handle. The belt started unrolling faster than expected and the IP grabbed the spinning handle to try and slow the belt down. The handle had a sharp edge which caused the cut. | |
3 November 2022 | OV | Harrogate | The IP was putting fondant into the boiling pan, as they did the fondant stuck to their glove and as the IP held the fondant over the pan and let go the fondant pulled off the IP’s glove, instinctively the IP reached out to grab the glove but in doing so got splashed on the thumb and middle finger with fudge. IP was provided with first aid care at the time of the accident, hospital treatment not required. once first aid care was given, the IP went back to work. | |
11 October 2022 | OV | Harrogate | The IP noticed that the Flying Scotsman cooling line belt had folded over itself, the IP (whilst wearing blue safety gloves) attempted to unfold the belt, whilst concentrating on fixing the belt the IP accidentally put his hands under the toffee flowing from the outlet pipe, toffee got inside his gloves and the IP on the right forearm and left hand. The IP had 1st aid treatment on site and then wanted the burns to be checked at A&E so Colin Clark took him. update – the IP has been advised by the hospital to keep his bandages clean and dry until he has a check up on Friday 14/10/22, will not be at work from 12/10/22 though till 14/10/22. | |

11 July 2022 | OV | Harrogate | The production on the Rocket line had just finished and the line was being washed. During washing the water is pumped from the boiling pan through the pipe to the holding pan and then through the another pipe into the waiting bath. The second pipe is usually positioned above the cooling belt to deposit product onto the belt. During washing, the pipe is turned approximately 60 degrees to the exit can be positioned above a bath to collect the dirty water. The Belt on the cooling tunnel for the line was due to be changed for a new belt due to damage sustained. OP34 had removed the old belt and was just preparing to fit the new belt. Due to the belt being 14m long it requires at least two people to fit the new belt to the line. The water from the holding pan had finished pumping into the bath when the new belt was being fitted. Fitting the new belt requires one person – in this case the IP, to hold the belt in place at the end of the cooling tunnel where the fudge is deposited onto the belt. The belt is then to be un-rolled by another person to then slide the belt over the roller at the opposite end of the cooling tunnel. | |
24 June 2022 | OV | Harrogate | The IP was pulling a pallet of honeycomb base (450kg) with a pallet truck through the coating room for coating, whilst next to the solid drain cover the IP’s feet slipped forward causing the IP to fall backwards onto his back. | |
23 June 2022 | OV | Harrogate | IP was processing toffee, the IP went to the end of the Scotsman line to get a euro bin of toffee to process, the bin closest to the Scotsman line was a full euro bin of toffee (100kg est) which was side on to the line, there was another euro bin in front of the 1st euro bin which was facing the 1st euro bin, the IP attempted to pull the 1st euro bin away from the line and as she did the euro bin hit some toffee that had gathered underneath the euro bin and caused the euro bin to twist, as this happened the IP’s finger got caught between the bin and the framework to the left of the Scotsman belt roller (the left as stood in front of the line looking down the cooling line towards the pans). | |

Because the machine is not plumbed into the hot water system and the pump is not very good at pumping water the pipe that feeds toffee syrup into the heat exchanger had become blocked with toffee. | |
